- VA Health Forms
- Parent Observation Form Rising Kindergartners Only
- Parish Registration
- 4. Submit the forms below to your current school:
- Release of Student Records
- Scholastic Form Rising Kindergartners Only
- Scholastic Form Rising Grade 1 Only
- Scholastic Form Rising Grades 2-7 Only
